Prasanna Santhanam 840da55520 api: Entities will extend InternalIdentity and Identity 
Entities correlated to the Identity and carry a uuid and those
correlated to InternalIdentity carry an id. Those entities that carry
both will correlated to Identity and InternalIdentity.

This refactors entities wherever possible to ensure the VO only
implements the first class entity.

1) Drop synchronized call semantic for ClusterManagerImpl.broadcast()
2) Have no choice now but to use an unbound thread pool to notify upper layer. This is to prevent thread starvation when we have cross-management server waitings.

Description:

1)	Moved RuntimeCloudException from api/ to utils/.

	Added simple constructor to RuntimeCloudException.

	Modified all classes that extended RuntimeException
	to extend RuntimeCloudException. These classes
	are listed below:

		CloudAuthenticationException
		CloudExecutionException
		AsyncCommandQueued
		HypervisorVersionChangedException
		RuntimeCloudException

2)	Added overloaded constructed to CloudException.

kishan c3ed426837 bug 12206: Added op_user_stats_log table to store historic user stats. Network stats update thread will update aggregate bytes and log the entry in this table. Mgmt server with min id will run this job.

Use a new target "system-integrity-checker" in components.xml/components-premium.xml.
All checkers must be explicitly specified in XML file, they will execute before any components load

* when management server dies and notifies other management servers about this, the running management server has to cleanup host_transfer records belonging to the died management server
* issue agent load balancing task only when agent load (number of connected agents in the system) exceeds "agent.load.threshold" - 70% by default
