This introduces new global settings to handle how client address checks
are handled by the API layer:
proxy.header.verify: enables/disables checking of ipaddresses from a
proxy set header
proxy.header.names: a list of names to check for allowed ipaddresses
from a proxy set header.
proxy.cidr: a list of cidrs for which \"proxy.header.names\" are
honoured if the \"Remote_Addr\" is in this list.

Most loggers didn't have to be static, so most of them were normalized so that they wouldn't be;
All loggers are protected now;
Static logger's name are now 'LOGGER';
Non-static logger's name are now 'logger';

This PR enables support for user data content upto 1048576 bytes - updates jetty maxFormContentSize value to 1048576 bytes (default is 200000 bytes).
CloudStack can support max user data content to 1048576 bytes (the size can be configurable through vm.userdata.max.length setting, max 1048576), but it's limited due to the default Jetty max content size, which is 200000 bytes.

Currently CloudStack is using logging frameworks as log4j and Java util logging, logging wrappers as slf4j and Apache common logging.
Here changes are to made it uniform, using only log4j framework.
Removed Java util logging, slf4j and Apache common logging.
Remove maven standard module (which only a few were using) and get ride of maven customization for the projects structure.
- moved all directories to src/main/java, src/main/resources, src/main/scripts, src/test/java, src/test/resources
- grep scan to search for src/com and src/org left over
- grep for <project>/scripts to fix pom.xml configuration
- remove custom <build> configuration in pom.xml
